ACCESS: Health Fairs

Health Screenings. Due to the reported lack of preventive care and increased risk for chronic illnesses within the Asian and Pacific Islander populations within the service planning areas, we have chosen to focus our health screenings on hypertension, diabetes, hepatitis B, and cholesterol. Undergraduate students certified by the American Heart Foundation will measure the client’s blood pressure certified medical students will perform finger stick blood glucose and cholesterol measurements and all clients will receive a hepatitis B titer per request. Blood samples will be collected by nurses and/or certified phlebotomists and clients will receive their results within 4 weeks.

Social Services. Trained undergraduate students will conduct social histories with clients. Undergraduate and public health students will perform eligibility screening with clients for enrollment in public benefit programs.

Health Education. Public health and medical students will provide health education to clients according to their specific needs and include recommended screening and early detection examinations, as identified through medical and social histories. Public health and undergraduate students will provide health education workshops on health issues specifically relevant to the Asian Pacific American community. All staff will develop language and culturally appropriate health education materials in conjunction with community based organizations and advisors.

Follow up. Client navigators will maintain contact with clients. Navigators will be undergraduate and public health students. They will ensure that clients follow up with recommendations, including obtaining clinical services, achieving enrollment and participation in any applicable public benefit programs, and achieving linkage with health resources. Client navigators will contact clients through telephone calls and mail correspondence. Public health students will manage client files, adhering to HIPAA (Health Insurance Portability and Accountability Act) guidelines. Clients with health care and social service needs that are beyond the scope of the project will be referred to appropriate resources in the community. Our primary referral will be the KORYO Health Foundation.
