  How do I apply?

1)
Complete online Volunteer Application

2) Go through online training for the Health Insurance Portability and Accountability Act (HIPAA)* and provide the temporary ID # of your certificate of completion when prompted in Volunteer Application

3) Print and sign the Waiver of Liability, Assumption of Risk, and Indemnity Agreement

4) Provide proof of immunization record**

5) Submit these three items together to one of the APHC directors (at a general meeting or during office hours at SAC 106E), or submit them to our mailbox at SAC 105: a) Waiver, b) Immunization Record, and c) $30 ($10 for a volunteer t-shirt and $20 deposit, which will be returned upon completion of your active membership expectations at the end of the year).

 


 
     
 

Volunteers will be trained and certified by the American Red Cross and medical doctors on safety measures and screening procedures. Volunteers who do not complete the above requirements will not be allowed to perform the screenings, but they can freely participate in other activities and leadership positions such as health fair planning.

*Health Insurance Portability & Accountability Act (HIPAA): All volunteers need to submit a HIPAA certificate (for patient confidentiality protection). Volunteers must understand that all information gathered from our screenings are to be kept strictly confidential. Collected information will be used solely for statistical purposes. To get HIPAA certified, volunteers must go to the MEDNET link provided above in item (2).

**Proof of Hepatitis B Immunization: Volunteers need to submit proof of Hepatitis B vaccination prior to performing any screenings. This can be a xerox copy of your yellow immunization card or a note from your family physician. If you need Hep B immunization, please contact your family physician or the UCLA ASHE center. This is mandatory from all volunteers to ensure that they have received the vaccination.

 
 


 
 


 
    What does membership entail?  
   


As an APHC member,
you must be firmly committed to the goals and objectives of the project. Members should also conduct all group-related activities in compliance with the policies and regulations of the University of California. Active membership entails attendance of:


1. 4 healthsites with at least one per quarter,
2. 3 general meetings per quarter,
3. 2 socials per year (one of which should be a culture night or conference), and
4. either 1 bagel sale per quarter or 1 TV taping per year.

Note: Volunteers must attend blood pressure and/or BMI trainings offered during fall and winter quarters and be certified in order to screen participants at healthsites.

   
 


 
     
   

As an APHC member, you will also...

+ Learn more about cardiovascular disease, obsesity, and cancer: During trainings and general meetings, volunteers will be given lectures on these health issues.

+ Interact with participants: At each site, trained volunteers will get the opportunity to educate and screen individuals.

+ Learn more about possible careers in the health field: APHC will invite speakers from various health fields to talk about their occupations. Members may also participate in our medical, dental, and optometry student mentorship programs.

+ Experience new cultures: Throughout the year, APHC screens participants from a variety of ethnic communities in Los Angeles.

+ Have leadership opportunities: APHC members will have numerous opportunities throughout the year to serve on various planning committees for health fairs co/sponsored by APHC.

+ Get letters of recommendation: APHC directors will take note of exceptionally dedicated volunteers and will make an effort to get to know all APHC members.

+ Get academic credits: You may be qualified to receive credits for you community service hours by enrolling in Civic Engagement (CE195s) with the Center for Community Learning.

+ Meet new people with similar goals and interests!
Every quarter APHC will organize various social events to allow members get to know one another while having lots of fun!
